Insurance

At this time, we are not able to bill any insurance plans as an in-network provider. We are considered out-of-network providers for every healthcare plan in California. 

We do not bill insurance directly and payment for services is due at the time of service. You can however submit a claims form with a superbill (given to you after the visit and payment is made in full). If insurance will reimburse, they will reimburse directly to you and not to the provider. 

Because of the vast number of insurance plans available, we do not know the specifics of your insurance plan. Though we can give you some guidance if requested, it is up to you to determine your insurance plan coverage.

HSA/Flex Benefits

We are able to accept HSA/Flex Benefit cards.

PPO/EPO insurances

We do not bill insurance directly. For PPO insurance plans, we are able to order blood work through insurance as long as the patient goes to a lab within their insurance network (usually Quest or Labcorp). Keep in mind that if you have a deductible for labs or a co-pay, it will still apply.

Office Visits: No in-network coverage. Payment due at time of service. Patient can submit a superbill (provided after the appointment is paid for in full) to insurance with a claims form for possible out-of-network coverage reimbursement directly to the patient.   There is an app that many find useful and easy for submitting insurance claims for reimbursement.  Go to https://reimbursify.com/individual-page/   for more information.

Labs: As long as patient goes to a lab within their insurance network (usually Quest or Labcorp), in-network coverage for labs. Deductible and copays still apply. 

Specialty labs: (ex: food sensitivity testing, adrenal testing, gastrointestinal testing). Coverage depends on the lab. Superbills can always be provided to submit to the lab for possible reimbursement directly to the patient. 

 

HMO/Medicare/Medi-Cal

For patients that have a HMO insurance, unfortunately we are not considered in-network or out of network. Labs and office visits are not covered. For bloodwork for HMO insurances, we are able to either give the patient a list of labs to take back to their primary care doctor to see if they will run it or the patient can get labs done through a discounted Labcorp service we offer.

Office Visits: No in-network  or out of network coverage. Payment due at time of service. 

Labs: No coverage; patient can see if a primary care doctor in their plan will order labs to go through their insurance, or we can give them an estimate for lab cost at the first visit. 

Specialty labs: (ex: food sensitivity testing, adrenal testing, gastrointestinal testing). No coverage by insurance. 

 

Tricare

For Tricare insurance, office visits are not covered as in-network or out-of-network. Payment is due at the time of service.

If the patient is active duty, we are not able to order labs. We are able to either give the patient a list of labs to take back to their primary care doctor to see if they will run it or the patient can get labs done through a discounted Labcorp service we offer.

For Tricare insurance for spouses, general blood work ordered through Quest/Labcorp has historically been covered. .  Tricare does not cover office visits.

Office Visits: No in-network or out of network coverage. Payment due at time of service. Tricare does not cover office visits.

Labs: Active Duty No coverage. Patient can see if a primary care doctor in their plan will order labs to go through their insurance, or we can give them an estimate for lab cost at the first visit. Spouse of active duty military, check with benefits. Historically general blood work ordered through Quest/Labcorp has historically been covered.

Specialty labs: (ex: food sensitivity testing, adrenal testing, gastrointestinal testing). No coverage by insurance
